Im 15 and looking for an application at some restaurants for a summer job, can someone give me a link to apply

I've looked all over the web for job applications and I cant find any! I just need a link to some applications for summer jobs like Arby's or KFC for a 15 year old. Also if you just happen to know the dress codes for the provided job, it would be appreciated so i would know what is appropriate to wear at work each day.
